School transition is a process that prepares students, families, schools and communities to develop the necessary skills, knowledge and relationships to assist students in successfully moving from one setting to another.
According to Child Mind Institute, dealing with change is often difficult for children, even if the change is just leaving the playground for the day or switching from art to math in school. For high school/college students, dealing with change can be life stage transitions like moving schools, starting or finishing university, starting a new job, adjusting to chronic illness, moving away from friends or family, etc.
If a child/teen struggles with a transition, it can have a negative impact on their well-being and academic achievement. During any transition period, it is important that children and young people can talk about their concerns and are supported to cope with any readjustments. Successful transitions are important to ongoing progress, lifelong learning and meaningful participation.
