BRIDGES from Inpatient to Outpatient Care

BRIDGES helps adults, youth and children transition from inpatient to outpatient care and/or are referred from COPES so that they continue getting better and lead happier and healthier lives.

Family & Children’s Services BRIDGES the gap because transitions from mental health-related hospitalization to outpatient care are often not successful. In fact, rates of outpatient non-engagement can range from 22 to 90 percent depending on multiple factors. This can affect a person’s ability to get on a path toward healing and reducing future hospitalizations.

Our Children/Youth Bridge Program wraps around not only the child/youth but the entire family so that the family’s needs are appropriately addressed and children/youth have everything they need to stay healthy and in their homes.

WHAT TO EXPECT

Your treatment team can assist your child and family in multiple areas of life by providing whole health care integrating physical, medical and behavioral health services including:

  • Primary care physician linkage and coordination
  • Medication management and pharmacy (psychiatric and medical)
  • Individual, family and group therapy
  • Advocacy in the school for behavior issues, suspensions, IEP and 504
  • Case management addressing complications effecting basic needs like food, housing, job and child care
  • Health promotion services
  • Advocacy in various systems including DHS and OJA
  • Referrals to community and social support services
  • Substance abuse treatment

ONGOING MENTAL HEALTH CHALLENGES

Hospitalization is often the first step for patients experiencing a significant mental health crisis, but aftercare is just as important for recovery. Our Adult Transitional Care Bridge Program helps individuals transition from inpatient to ongoing outpatient care.

  • Case Manager/Care Coordinator

  • Peer Recovery Support Specialist
  • Wellness Coach
  • Primary Care Physician Consultant
  • Medication Clinic Team

WHAT TO EXPECT

Upon dismissal from a hospital or psychiatric care center, patients will develop an ongoing plan for care with their therapist or case manager.

That first appointment is the first step in ongoing management of positive mental health.

Your treatment team can assist you in multiple areas of life by providing whole health care integrating physical, medical and behavioral health services including:

  • Primary care physician linkage and coordination
  • Medication management and pharmacy (psychiatric and medical)
  • Individual, family and group therapy
  • Case management addressing complications effecting basic needs like food, housing, job and child care

ADULT BRIDGE PROGRAM BENEFITS

  • Reduce barriers to seeking outpatient care
    • This may include reminders, transportation to appointments or helping individuals obtain technology for home-based virtual appointments.
  •  Ongoing management of mental health challenges
  • Maintain stable functioning
  • Continue path toward recovery
  • Reduce future hospitalizations
  • Reduce risk of non-adherence to medications, relapse, symptoms exacerbation and suicide
